Insurance

In-network insurance includes, but is not limited to, Anthem, Kaiser, Medical Mutual of Ohio, QualChoice. Co-pays are the client’s responsibility. We are a Bureau of Worker’s Compensation certified provider.

Medicare is accepted as an insurance provider as well. Any deductible is the responsibility of the client.

Out-of-network providers generally pay 60% - 70% of the cost. The balance is the responsibility of the client. Out-of-network providers include, but are not limited to, Aetna, Cigna, Cleveland Health Network, Emerald Health, and United Healthcare.
